Surrey Care Record
What is the Surrey Care Record?
The scheme enables the sharing of Health and Social Care Records with other NHS local Surrey A&E departments such as A&E at The Royal Surrey County Hospital, St Peter’s Hospital, and Epsom Hospital. This means that important information can be obtained quickly by Care Professionals when they need it to ensure you are given the best possible care.
Care Professionals will also tell you when they intend to access your Surrey Care Record at the point of care (for example, on arrival at A&E) giving you another opportunity to decide if you agree or not.
Information is not shared with any other private medical organizations.
Benefits of the Scheme
Benefits are:
- Improved quality of care – information about your care will be instantly available to Care Professionals for more accurate diagnosis and ongoing treatment.
- Improved patient safety – there will be greater visibility for your health and social providers about your current medications, allergies, and adverse reactions.
- Reduced delays in care – test results will be readily available which reduces patient waiting time.
You have a Choice
Although there are clear benefits to your care, you can, nonetheless, opt out of the Surrey Care Record Scheme so that your health and social care records are not shared with other NHS local Surrey hospitals. We do not recommend you do this because of the benefits to you but you have the right to opt out.
If you wish to opt out, please choose this option on the Registration Form but, in so doing, you should be aware that this could adversely affect your care in the circumstances indicated above.
We hope that you wish to participate in the scheme and, if so, please choose the “Opt In” selection.
If you select neither option, we will assume that you wish to participate in the scheme and will opt you into the Surrey Care Record Scheme on your behalf because of the potential benefits to you.
